Date of Incident:

10-31-79 2.

Identification of specific effluent limitation exceeded:

Monthly average limitation of 15 mg/l for oil and grea_sm>.

Description of Incident:

For the month of October, five samples were collected with a daily average of 21.9 mg/1.

4.

Apparent Cause:

The problem was apparently caused by a failure of the oil separator or improper operation.

5.

If not immediately corrected, the anticipated time the condition is expected to continue:

NA 6.

Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:

The separator was restored to proper operation and subsequent analysis shows results in the ?-10 mg/l range after 11/1/79.

Daily maximum limitation of 20 mg/l for oil and grease.

Description:

Laundry Wastes Prior to Dilution Outfall No 800174 Parameter: Phosphorus - Total Parameter No 00665 1.

Date of Incident:

10-20-79 2.

Identification of specific effluent limitation exceeded:.

Daily maximum total phosphorus limitation of 2.0 mg/1.

3.

Description of Incident:

On 10/20/79 a total of 5,000 gallons of laundry water with a total phosphorus concentration of 2.58 mg/l was released.

4.

Apparent Cause:

A change in the type and quantity of detergent used occurred at this time.

5.

If not immediately corrected, the anticipated time the condition is expected to continue:

NA 6.

Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:

Adjustments were made in the quantity of soap used. A possible problem with the testing reagents is also being evaluated.
