CAUSE DESCRIPTION AND CORRECTIVE ACTIONS h 1 O l The failure of the HPCI Steam Line Inboard Valve to isolate was due to the torque l i i i I switch being set on a conservative setting. The engineering package for the valve was j i 7 l reviewed and it was determined that a higher setting could be used to provide more l 3 l reliable closing of the valve. The torque switch was set to the new value and the l The valve was then cycled and found to be operating (Copt) i 4 l valve placed back in service. 80 7 8 9
