INSERVICE INSPECTION-EXAMINATION

SUMMARY

PRAIRIE ISLAND NUCLEAR GENERATING PLANT-UNIT I JULY 11 TO JULY 31, 1979 l.0 INTRODUCTION This report is a summary of the examinations performed during a turbine outage at the Prairie Island Nuclear Generating Plant -

Unit I.

This was the second special inservice inspection con-ducted on feedwater systes piping welds during inspection period two.

The examinations were performed during a turbine outage from July 11 to July 31, 1979. Prairie Island-Unit I began com-mercial operation on December 16, 1973.

The examinations were performed on pressure retaining components and there supports of the reactor coolant and associated auxiliary systems classified as ASMF Class 1 and Class 2.

The majority of the examinations were performed on the feedwater system piping welds to comply with IE Bulletin No. 79-13.

The feedwater pipe to steam generator nozzle welds, that were examined during the June 1979 weekend outage, were reexamined this outage to comply with the additional requirements specified in the Bulletin.

The evaluation of all the results from the inservice examinations conducted during the Prairie Island-Unit I outage indicated that the integrity of these systems has been maintained. There were no indications of degradation to t.rse systems.

The components examined, examination method and results are summa-rized in the tables of the attached appendices as follows:

- No examinations scheduled this outage 2.0 DISCUSSION OF EXAMINATION PLAN 2.1 Inspection Boundary The examination plan focused on the feedwater and auxiliary feedwater piping welds and supports classified as ASME Class 2.

This plan was based on the requirements of IE Bulletin No. 79-13.

and radiographic were performed on all feedwater system cir-cumferential butt welds from containment penettation on loop A and from the end of the encapsulation sleeve on loop B to and including the pipe to steam generator nozzle welds. The examinations were also performed on the auxiliary feedwater sweep-o-let conneccion weld to the main feedwater lines and on the pipe base metal from the auxiliary feedwater connec-tion to the pipe to steam generator nozzle weld.

In addition, a visual (on feedwater loop A and B) and magnetic particle (on loop A) examination was performed on all accessible restraints and supports inside containment.

Examinations were also performed on pressure retaining compo-nents and there supports of the reactor coolant and associated auxiliary systems classified as ASME Class 1.

These examina-tions were based on the requirements of the ASME Boiler and Pressure Vessel Code Section XI, 1974 Edition through and in-cluding the Summer 1975 Addenda, and complied with Prairie Island's Technical Specification, Section TS 4.2.

The exami-nation is in accordance with the program submitted to the United States Nuclear Regulatory Commission on February 1,1978 enti-tied, "ASME Code Section XI Inservice Inspection and Testing Program and Information Required for NRC Review of Requests for Relief from ASME Code Section XI Requirements".

2.2 Examination Methods Ultrasonic and radiographic examination methods and techniques were used to perform tne volumetric examinations. The ultra-sonic test system consisted of an ultrasonic digital analog tester and a two channel strip chart recorder. One channel of the recorder was calibrated to reflect ultrasonic screen height (amplitude) and the second channel was calibrated to indicate metal path (range) to the reflector. This approach to the examination gives a permanent record to the extent possible.

The radiographic examinations were performed using a double wall exposure, single wall viewing technique. An Iridium 192 source was used in conjunction with multiple Kokak "M" film.

In addition, the radiographic examinations were performed to the 2T penetrameter sensitivity level; and, where geometry permitted, for a distance equal to two wall thickness on each side of the weld.

Liquid penetrant or magnetic particle examination methods were used to perform the surface examinations.

The liquid penetrant examinations were performed using color contrast-solvent remov-able materials. Magnetic particle examinations were performed using a yoke and dry powder.

All visual examinations were aided when necessary with artifi-cial lighting and verified for adequacy with an 18% neutral grey card with a 1/32 inch black line, b s $;" C @

2.3 Examination Procedures The ultrasonic examination procedure for pipe welds comolied with the requirements of Appendix El of ASME Section XI that was issued in the Winter 1975 Addenda. The radiographic ex-amination procedure complied with the requirements of ASME Section III,1974 Edition through and including the Winter 1976 Addenda. In addition, a field change was issued to this procedure to require 2T penetrameter sensitivity in lieu of the 4T essential hole referenced by the procedure. All other examination procedures complied with the requirements of the 1974 Edition through and including the Summer 1975 Addenda of ASME Section XI.

A listing of the procedures used for the examinations is shown in Table III of Appendix E.

2.4 Equipment and Materials All equipment and expendable materials used in the examina-tions are listed by either serial number or type along with their respective calibration date or batch number in Table IV of Appendix E.

The ultrasonic calibration standards used in the examination are listed in Table II of Appendix E.

These standards are owned and maintained by NSP at the plant site.

2.5 Personnel Northern States Power Company contracted Lambert, MacGill, Thomas, Inc. and Peabody Testing Services to perform the examinations. Hartford Steam Boiler Inspection and Insurance Company, representing ANI, provided the Authorized Inspection.

All personnel involved in the performance or evaluation of examinations are listed along with their title, organization, and ASNT Level of certification in Table I of Appendix E.

Certifications for examination personnel are maintained on file by Northern States Power Company.

2.6 Evaluation Any indications disclosed in the examinations were evaluated by the examiner at that time, in accordance with the rules of the procedure and ASMF Section XI.

The ultrasonic examiner was aided in his evaluation by a cali-bration performed on a standard reference before each day's examination, checked before and after each individual examina-tion, and at intervals not exceeding four(4) hours.

In addition, the ultrasonic data was recorded on strip charts which were made a part of the inspection report, and permitted further evalua-tion.

2.7 Examination Reports and Documentation All examination reports and documentation are maintained on file by Northern States Power Company. Table I of Appendix A and B identified the examination report number (s) for each item examined. Many of the items identify more than one re-port number because of the different types of examinations performed on them.

Table I of Appendix A and B summarized all the examinations that have been performed to date and identifies the amount that will be examined in the future to complete the ten year examination requirements. For retrieval purposes, the prefix of the inspection report number corresponds with the year that the inspection was performed. The examination reports for this outage are prefixed with "79-SP2".

Table II of Appendix A and B compares the baseline results to those obtained during this examination. Table III of Appendix A and B identifies the isometric drawings that were used for the examinations. The personnel, ultrasonic calibration blocks, procedures, equipment and materials that were used are identified in the tables of Appendix E.

Appendix F contains the Form NIS-1, entitled " Owners' Data Report for Inservice Inspections".

3.0

SUMMARY

OF RESULTS The results of these examinations indicated that the integrity of these systems were maintained.

There was no indication of cracking in any of the areas examined.

The visual examinations revealed two loose nuts on two feedwater loop A restraints and one loose bolt on a Pressurizer Surge line hanger. The liquid penetrant examinations revealed one linear indication on a socket weld on RTD Take Off Hot Leg Loop A.

The magnetic particle examinations revealed, in four locations, linear surface irregularities. The radiographic exami-nations revealed the same indications as would be seen on the con-struction radiographs, i.e., porosity and slag, including one elongated slag inclusion and wormhole. The only indications re-vealed by ultrasonic examinations were geometric reflectors and a small linear indication in the same area as the elongated slag in-clusion.

All indications disclosed during the examinations were resolved prior to plant heat up.

The loose nuts and bolts on the restraints were tightened. The magnetic particle linear surface irregularities in the four areas were removed by light hand grinding and blending the surface smooth. The elongated slag inclusion and wormhole in weld FW-159 was determined to be unacceptable to the ASME Section III Code. The slag and wormhole was removed by grinding and the area was repair welded. The original construction radiographs were reviewed and compared to the inservice radiographs.

It was determined that the indication had nat propagated; but, that the higher sensitivity inservice examination core clearly defined the discontinuity.

The liquid penetrant indication was also removed by light hand blending.

All indications were corrected and the areas were reexamined with the same method that located them.

Abstract of Examinations.

Include a list of examinations and a statement concerning status of work required for current interval.

Examinations were performed on all feedwater system circumferential butt welds from containment penetration on loop A and from the end of the encapsulation sleeve on loop B to and including the pipe to steam generator nozzle welds. The examinations were also performed on the auxiliary feedwater sweep-o-let connection weld to the main feedwater lines and on the pipe base metal from the auxiliary feedwater connection to the pipe to steam gererator nozzle weld. Examinations were also perfomed on all accessible supports and restraints on feedwater loop A and B inside con-tainment. The remainder of the examinations completed approximately 5% of the required ASME Class 1 examinations.

11.) Abstract of Conditions Noted. The results of these examinations indicated that the integrity of these systems was maintained. There was no indication of cracking in any of the areas examined. The only anomalies detected were: one loose bolt on feedwater A, restraint T; two loose nuts on feedwater A, restraint X; one loose bolt on Pressurizer Surge, hanger D; linear MT surface indications in four locations; one linear PT indication I

on RTD Take Off Hot Leg A, socket weld SW-10; and an elongated slag and a wormhole indica-tion on FM-159.

12.)

Abstract of Corrective Measures Recommended and Taken. All indications disclosed during the examinations were resolved prior to plant heat up.

The loose bolts and nuts on the restraints were tightened. The linear surface irregularities in the four areas were removed by light hand grinding and blending the surface smooth. The elongated slag ano wormhole in weld FW-159 was removed by grinding and the area was welded up.

The PT indication was also removed by light hand blending. All indications were corrected and the areas were reexamined with the same method that located them.

We certify that the statements made in this report are correct and the examinations and corrective measures taken conform to the rules of the ASF1E Code,Section XI.
