Letter from G. S. Spencer to N. O. Strand, fiRC Inspection WNP-1 & 4, dated June 22, 1979 This is in response to the reference letter which discussed the results of the April 24-27, 1979 inspection of activities authorized by NRC Construction Permits Nos. CPPR - 134 and 174.

The reference letter identified a certain activity which was not conducted in full compliance with PSAR requirements, as set forth in the Notice of Deviation, enclosed as Appendix A.

This item of noncompliance has been categorized into a level as described in your correspondence to all NRC licensees dated December 31, 1974.

CPPR - 134 and 174 Notice of Deviation Based on the results of an f4RC inspection conducted at the WNP-1 site on April 24-27,1979, it appears that one of your activities was not conducted in accordance with your commitments to the Commission as indicated below:

Contrary to the commitment in Sections 3. 2. 2. 3, 3. 2. 4.1, a nd Table 3.2-2 of the PSAR, a pipe spool classed as ASME Section III, Class 3 (WPPSS Quality Class I) for the Containment Equipment and Floor Drains (DFR system), was found to contain a field fabricated weld which did not meet the requirements of ASME Section III, Class 3 as stated in Section ND 5300 of the ASME B&PV Code.

In this weld, the inspector observed that three or more inches of the weld root lacked complete fusion. The weld (No. 500 of the DFR System) had been accepted.

Supply System Response Action Taken to Correct the Identified Item The Contractor initiated nonconformance report number 1-CNCR-211-005 on April 26, 1979 to document the discrepant weld.

Weld number 500, spool piece number DFR-412088-4 was repaired and subsequently accepted on May 10, 1979.

Action Taken to Preclude Recurrence The Contractor conducted a training session on May 4,1979 to reiterate that all defective welds will be documented in a Cf!CR.

Inspectors are to assure that internal surface of the root pass is acceptable, as per Visual Inspection Procedure JAJ-NDE-008.

Date of Full Compliance Full compliance was achieved May 10, 1979.
