CORRECTIVE ACTIONS

Contacted Security and Operations to reset pins and close door. Performed a walk down of the area. The airlock doors are now posted as "Emergency Exit Only" until the reactor building and auxiliary building door interlock is repaired.

A Human Performance Department Reset was issued as a result of this event. The event briefing sheet was presented to the Radiation Protection department staff. Radiation Protection Management communicated to the staff the importance of ensuring the first door is closed prior to opening the second door, and not challenging the interlock.

The interlocking device is being reviewed. When the appropriate design or adjustment is determined, the interlock will be repaired.

PREVIOUS OCCURRENCES

The negative pressure in SC was maintained by Reactor Building Heating Ventilation and Air Conditioning (RBHVAC)

((VA)) and remained within the TS requirement during this event. There are no previous occurrences that involved the human performance error of simultaneous opening of both air lock doors.Page 3
