NON-RADIOLOGICAL NPDES PERMIT VIOLATION REPORT NOVEMBER 1991 THROUGH APRIL 1992 During the period of November 1, 1991 through April 30, 1992 there were two NPDES violations reported to the Michigan Department of Natural Resources (MDNR).
The two NPDES violations are summarized below.
- 1.
On December 10, 1991, the plant exceeded the maximum oil and grease concentration to the plants mixing basin (Outfall 001).
The sample from the Turbine Sump Outfall Tank contained 58.4 ppm oil and grease, which exceeded the maximum permitted limit of 20 ppm.
- 2.
On April 8, 1992, the plant exceeded the maximum oil and grease concentration to the plants mixing basin (Outfall 001).
During routine analysis of the turbine sump outfall tank, a sample contained 25.6 ppm oil and grease. A second sample was obtained to verify the results. The second sample contained 30.6 ppm oil and grease.
The maximum limit of oil and grease to Outfall 001 is 20 ppm.
The NPDES notification reports to MDNR for both NPDES violations are attached.
In the same period no reports of Oil, Salt or Polluting Material loss were required.
Other activities that may be of interest pertaining to water quality are summarized below.
- 1.
On January 24, 1992, permission was received from the MDNR to use and release a dye, Rhodamine WT.
This dye will be used to trace a leak in the storm drain from the top of the turbine building to the 590' elevation of the turbine building.
- 2.
On February 25, 1992, Palisades received permission from the MDNR to
~oute the main plant transformer sump stormwater to Outfall 001.
During periods of heavy rainfall the main plant transformer sump will collect a substantial amount of rainwater.
Prior to batching the sump, a PCB analysis will be performed to verify no PCBs are present in the stormwater.
- 3.
On April 3, 1992, during the performance of a special test which checked the operation of two valves associated with the component cooling water (CCW) system, an inadvertent release of 160 gallons of CCW occurred.
The valves, when opened, allowed water containing sodium nitrite to be released into the service water system, which discharges into Lake Michigan.
A nitrite analysis of the service water discharge was performed immediately following the release.
The results of the analysis indicated less tan detectable levels of nitrites present in the sample.
This event did not result in an NPDES violation, but was reported to the MDNR.
The report served as a notification of releasing a chemical which is not authorized within Palisades NPDES permit.

Description of Incident:
As a result of turbine.lube oil present in the turbine building sump, a sample of the turbine sump oil separator tank (T-41) on December 10, 1991 measured 58.4 mg/l oil and grease.
- 4.
Apparent Cause:
Lube oil was apparently introduced into the turbine sump via decanting of the lube oil coaleser.
As water was being drained from the coaleser, some lube oil may have been inadvertently introduced into the turbine sump.
- 5. If not iIIDilediately corrected, the anticipated time the condition is expect-ed to continue:
Corrective action was initiated iIIDilediately upon identification of this problem.
- 6.
Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:
Corrective action included pumping of the turbine sump, M-18 oil/water separator and T-41 by a licensed waste hauler.

Additional Comments:

IIIDilediate observations of the Plant's mixing basin (Outfall 001) and Lake Michigan showed no oil present.

Description of Incident:
A sample of the turbine sump oil separator tank (T-41) on April 8, 1992 measured 25.6 mg/l oil and grease.
A repeat sample measured 30.6 mg/l oil and grease.
- 4.
Apparent Cause:
Indeterminate at this time.
The M-18 oil/water separator was checked and determined to be operating properly and had not been bypassed.
No other sources or unusual operating condition could be determined.
- 5. If not immediately corrected, the anticipated time the condition is expected to continue:
Corrective action was initiated immediately upon identification of this problem.
- 6.
Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:
Oil absorbant pillows were placed in the Turbine Building Sump.and replaced in tank T-41.
- 7.
Additional Comments:
- 8.
Immediate observations of the Plant's mixing basin (Outfall 001) and Lake Michigan showed no oil present.
This matter is believed to have been an isolated event which is not expected to reoccur.
An internal corrective action document was written to initiate and document this event and also to make the necessary corrective actions.
