APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Docket No. 50-311 This refers to the inspection conducted by a representative of the Region I (Philadephia) office at the Salem Nuclear Generating Station, Hnit 2, Hancocks Bridge, New Jersey, of activities authorized by NRC License No. CPPR-53.-
During this inspection, conducted on February 13-14, 1979, the following appa.rent item of noncompliance was identified:
10 CFR 50, Appendix B, Criterion V states, in part, 11Activities
_affecting quality shall be prescribed by documented instructions...
and sha*ll be accomplished in accordance with these instructions...
11 Salem FSAR, Section D.1-22 states, in part, 11 *** Activities that affect quality are prescribed by PSE&G and 'its contractors by means of written procedures, instructions, dr_awings, and.check_ Ti sts....
11 Detail Construction Procedure (DCP) 2205, Rev. 9, paragraphs 4.23 and 4.24 states in part, 11 No craft personnel shall walk in trays except when absolutely necessary to pull cable and cable trays shall be maintained free of debriS, construction materials, tools, etc. 11 Contrary to the above, on February 13, 1979, craft personnel were observed walking on safety related cables in safety related tray 2P037.
- Trays ~P027 and 2P066 were observed to be distorted and bent apparently from the application of excessive loads. *Further, there were metal shavings debris on and between the cables in tray 2P027.
