Dear Mr. Grier:
NRC INSPECTION REPORT NO. 50-311/79-12 NO. 2 UNIT SALEM NUCLEAR GENERATING STATION May 4, 1979 We have reviewed the report of your inspection conducted on February 13-14, 1979, which was transmitted with your letter of April 6, 1979, and received on April 9, 1979.
Our response to the item of noncompliance is as follows:
"10 CFR 50, Appendix B, Criterion V states, in part tActivities affecting quality shall be prescribed by documented instructions
... and shall be accomplished in accordance with these,instruc-tions... '
I "Salem FSAR, Section D.1-22 states, in part,. '... Activities that affect quality are prescribed by PSE&G and its c6ntractors by means of written procedures, instructionsj drawings, and check lists
" Detail Construction Procedtire (DCP) 2205, Rev. 9, paragraphs 4.23 and 4.24 states in part, 'No craft personnel shall walk in trays except when absolutely necessary to pull cable and cable trays shall be maintained free of debris, construction materials, tools, etc.'
"Contrary to the above, on February 13, 1979, craft personnel were observed walking on safety related cables in safety related tray 2P037.
Trays 2P027 and 2P066 were observed to be distorted and bent apparently from the application of excessive loads.
Further, there were metal shavings debris on and between the cables in t~ay 2P027."

5/4/79 The following corrective measures have been initiated:
- 1.
Upon identification of the cited noncompliance, Site Quality Assurance issued Corrective Action Request (CAR)
No. SC-376-0 to our Construction Manager (CM), UE&C.
- 2.
On February 13 and 14, 1979, verbal and written instructions were issued by the UE&C Project Superintendent to all craft supervisors and General Foremen emphasizing the importance of enforcing the requirements of DCP 2205.
In addition, signs were posted in conspicuous areas of the plant which read as follows:
"STANDING/WALKING ON* CABLE TRAYS WILL NOT BE TOLERATED."
- 3.
The subject cable trays were cleaned to remove metal shavings and other debris.
Tray 2P027 was replaced and tray 2P066 was reworked.
UE&C Quality Control and PSE&G Site QA have monitored subsequent field activities and no other incidents of the subject nature have been reported.
- 4.
We are in_ full compliance now.
