ENCLOSURE I NOTICE OF VIOLATION Virginia Electric and Power Company Surry Unit I and 2 Docket No. 50-280, 50-281 License No. DPR-32, DPR-37 During the Nuclear Regulatory Corrmission (NRC) inspection conducted on May 10 through June 6, 1992, a violation of NRC requirements was identified.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violation is listed below:

IO CFR 50.59(b)(l) requires, in part, that records of changes in procedures as described in the safety analysis report include written safety evaluations which provide the basis for the determination that the procedure changes do not involve unreviewed safety questions.

Contrary to the above, written safety evaluations were not performed for procedures that operated plant systems differently than described in the Updated Final Safety Analysis Report.

Examples of such procedures were:

OP 52.2.1, Administrative Control of l-FP-36, dated October 27, 1989;

  • 2-0P-49.7, Filling and Draining RSHX Service Water Supply Piping, dated September 18, 1991;-and OP 6.2.3, Administrative Control of l-EG-15, 2-EG-15 or 3-EG-15, dated January 20, 1990.

This is a Severity Leve.l IV Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Virginia-Electric and Power Company is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region II, a copy to the NRC Resident Inspector, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a "Reply to a Notice of Violation" and ~hould include for each violation:

(1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.

If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked, or*why such other action as may be proper should not be taken.

Where good cause is shown, consideration will be given to extending the response time.
