ENCLOSURE 1 NOTICE OF VIOLATION Virginia Electric and Power C001pany Surry Unit 2 Docket No. 50-281 License No. DPR-37 During the Nuclear Regulatory Commission (NRC) inspection conducted between the
. period of October 1 to October 28, 1989, a violation of NRC requirement~ was identified.
In accordance with the "General Statt:nient of Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1989), the violation is listE:d below:
A.
Technical Specification 6.4 requires, in part, that detailed written procedures with appropriate checkoff. lists and instructions sha 11 be provided and followed for operations and testing of systems and components involving nuclear safety.
Contrary to the above, procedures arid instructions were not followed in that th~ following:
- 1.
On October 8, 1989, testing of Unit 2 train B auxiliary feedwater pump 2-FW-P-38 was not performed in accordanc~ with periodic test procedure 2-PT-15.18 resulting in an inadvertent isolation of the train A pump, 2-FW-P-3A.
- 2.
On Octobe_r 25, 1989, licensed operators failed to comply with procedure 2-0P-5.1.1 and previous training instructions resulting in an unplanned dilution of the Unit 2 reactor coolant system without proper establishment of containment integrity, a* violation of Technical Specificaticin 3.8.A.6.
This *is a Severity Level IV violation (Supplement I) and applies to Unit 2 o~ly.
Pursuant to the provisions of 10 CFR 2.201, Virginia Electric and Power Company is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector, within 30 days of the date of the letter transmitting-this Nati ce.
This reply should be cl early marked as a "Reply to a Notice of Violation" and should include:
(1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved; (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.
Where good cause is ~hown, consideration will be given to extending the response time.
