(NRC INSPECTION REPORT NOS. 50-280/89-24 AND 50-281/89-24)
This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by
W. E. Holland, L. E. Nicholson, and J. W. York on July 30 through September 2,
1989.
The inspection included a review of activities authorized for your Surry
facility.* At the conclusion 6f the inspection, the findings were discussed
wi~h those members of your staff identified ih the enclosed inspection report .
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of selective examinations of procedures
and representative :records, interviews with personnel, and observation of
activities in progreis.
The inspection findings indicate that certain activities appeared to violate
NRC requirements.
The violations, references to pertinent requirements, and
elements to be included in your response are described in the enclosed Notice
of Violation.
The enclosed inspection r~port also describes apparent violations relating to
high radiation area access controls and maintenance activity corrective
actions, which are under consideration for escalated enforcement .action.
Accordingly, no Notice of Violation is being issued at this time for the
apparent violations in these areas.
However, please be advised that the
characterization of these apparent violations described in the enclosed
inspection report may change as a result of further NRC review.
An
Enforcement Conference to discuss these apparent violations has been scheduled
for October 17, 1989.
You wi 11 be advised by separate correspondence of the
results of our deliberations on these matters.
In addition, a weakness in your threshold for reportabil ity of events in
accordance with 10 CFR 50.72 is discussed in the enclosed inspection report.
Although we concluded that this issue did not warrant issuance of a violation,
we consider that the process for determining reportability of events was
deficient.
We believe that you and your staff have taken appropriate action to

adjust the reportability threshold for 10 CFR 50.72 events.
Your response to
future events will be closely monitored to verify successful resolution of this
problem.
In accordance with Section 2.790 of the NRC's "Rules of Practice," Part 2,
Title 10, Code of Federal Regulations, a copy of this letter and its enclosures
will be placed in the NRC Public Document Room.
The responses directed by this letter and its enclosures are not subject to the
clearance procedures of the Office of Management and Budget as required by the
Paperwork Reduction Act of 1980, Pub. L. No.96-511.
