ENCLOSURE 1 NOTICE OF VIOLATION Virginia Electric and Power Company Surry 1 and 2 Docket Nos. 50-280, 50-281 License Nos. DPR-32, DPR-37 During the Nuclear Regulatory Commission (NRC) inspection conducted on.
March 27 - April 4, 1989, a violation of NRC requirements was identified.
In accordance with the "General Statement of Pol icy and Procedure for NRC Enforcement Actions, 11
- 10 CFR Part 2, Appendix C (1989), the violation is listed below:
10 CFR 50 Appendix B, Criterion XVI and the licensee's accepted Quality Assurance program (VEP l-5A, Section 17.2.16) collectively require that measures be established to assure that conditions adverse to quality are promptly identified and corrected.
In case of significant conditions adverse to qua 1 i ty, the measures shall assure that the cause of the condition is determined and corrective action taken to preclude re pet it ion.
Contrary to the above, measures were not adequate to assure conditions adverse to qua 1 i ty were corrected nor the cause of the condition determ1ned for problems identified by the 1 icensee with valves l-CH-MOV-1286B in 1988 (inadequate actuator grease), 2-CH-MOV-2289A in
- 1986 (damaged valve disk) and 2-CH-MOV-22898 in 1987 (excessive valve stroke times).
These valves are included in the licensee's IE Bulletin 85-03, Motor Operated Valve Common Mode Failure During Plant Transients Due to Improper Switch Settings program.
This is a Severity Level IV violation (Supplement I).
Pursuant to the provisions of IO CFR 2.201, Virginia Electric and Power Company is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN:
Document Control Desk, Washington, DC
- 20555, with a copy to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector, Surry, within 30 days of the date of the letter trans-mitting this Notice.
This reply should be clearly marked as a "Reply to a Notice of Violation" and should include [for each violation]:
(I) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending the response time.
