APPENDIX A NOTICE OF VIOLATION Virginia Electric and Power Company Surry Docket No. 50-280 License No. DPR-32 As a result of the inspection conducted on July 3-8, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified.
Technical Specification 6.4.E requires that a temporary change to a periodic test procedure may be made provided the change is approved by the Cognizant Supervisor and a licensed Senior Reactor Operator prior to implementing the change.
Contrary to the above, Step E.3.2 of Periodic Test 28.11, Control Bank B Rod Worth Measurement Using Dilution, was deleted without documented prior approval.
Water samples for analysis of the boron concentration in the reactor coolant system and pressurizer were not taken at 15 and 30 minute intervals as specified by Step E.3.2.
Failure to obtain the samples at 15 and 30 minute intervals did not invalidate the test results.
This is a Severity Level VI Violation (Supplement I.F).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) admission or denial of the alleged viola-tion; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
