Our response to the specific infraction is attached, We have determined that no proprietary information is contained in the reports.

Accordingly, the Virginia Electric and Power Company has no objection to these inspection reports being made a

matter of public disclosure.

The information contained in the attached pages is true and accurate to the best of my knowledge and belief.

e Attachment Page 1 RESPONSE TO NOTICE OF VIOLATION APPENDIX A TO I.E. REPORTS 50-280/81-17 AND 50-281/81-17 As a result of the inspection conducted on May 1-29, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified.

Technical Specification 3.21.G requires that all penetration fire barriers protecting safety related areas be functional, otherwise a continuous fire watch shall be established within one hour.

Fire doors serve as fire barriers and must be functional.

NFPA-80, Section 516.b requires that all swinging type fire doors be provided with approved positive latching devices to be functional.

Contrary to the above, on May 28, 1981, the following fire barriers (swinging type fire doors) were in access and not functional due to inoperable latching devices, and a continuous fire watch was not properly established:

Fire doors 37, 39, and 41 leading from the mezzanine into the three emergency diesel generator rooms, Fire door 21 between the turbine building and the emergency switchgear room, Fire door 10 into the cable spreading room above the control rooms.

This is a Severity Level V Violation (Supp. I.E.).

RESPONSE

1.

The item as stated is correct.

2.

Reason for Violation:

An extremely hard rainstorm had rendered the normal latch on the fire doors in question unresponsive to key cards.

At the same time, the heavy rain had created other flooding hazards which the Shift Super-visor on duty felt it was his first priority to cope with.

He therefore ordered the combination fire door/security door placed in access by Security in order to allow free access for Operations personnel should it be needed to handle operational emergencies.

Corrective Steps Which Have Been Taken and the Results Achieved:

The card key latched doors were removed from access (made operable) or a continous fire watch was established at the doors where personnel access was required.

Corrective Steps Which Will be Taken to Avoid Further Violations:

A Security Procedure will be issued for placing card key latched ~oors in access.

The Procedure will require that the door*will only be placed in access when personnel are present at the door or are in the space and can function as a fire watch.

Instructions have been issued reemphasizing the requirement that a fire watch must be posted when a fire door is blocked open.
