NON-COMPLIANCE with FIRE PIDTECTION 'Pl~EDURES

1. DEOCRIPI'ION of EVENT:

With Unit 1 defueled and Unit 2 operating steady state at 100% p::>wer, PI' 24.11 was performed disclosing 21 improperly sealed fire barrier pene-trations in safety related areas (Safeguards, Turbine Building, Aux. Build-ing and Service Building).

Fire watches were'not estabiished within one hour as reqw.red*by'i'ech~ Spec.

3.21.G.2.2.) Thus is re:i;nrtable.as per T.S. 6.6.2.b. (3).

PIDBABLE CON~S and STA'IUS of REDUNDA..'l'Il' SYSTEMS:

The functional integrity of the fire barrier _penetration seals ensures' that fires will be confined or adequately retarted from spreading to adjacent

i;nrtions of the facility. Establishing a fire watch adjacent to non-functional fire barrier penetrations insures pranpt detection of a fire before it can spread.

Because various penetrations were rot properly sealed and fire in one area could :i;nssibly spread through the no:rmal fire barriers. All other fire det~tion and mitigation systems renained operable and would_ have serve-:!.

to_ provide the early de~tio~ an:l *requ1ra:1. *fife* *.£ighting action.

  • Therefore*the health and safety of the general p..iblic were mt affecte1.

CAUSE

Fire watches supj;x:>r.ting' ** construction in the eff~tive. area of the barriers were erroneously assume::i to satisfy the requirements.

Further evaluation detennined that a special set of fire watches were required. These fire watches were not established within one hour as require-:!. by.T.S. 3.21.G.2.a.

Established fire watches in all safety relate-:!. areas where def~tive penetration seals were identifie1.

Defective penetrations were sealed with the.required material using approved proca:iures.

ACTION TAKEN to PREVENI' REXlJRREN:E:

Periodic Test 24. ll is being revised to strengthen proce'.3.ures

  • Personnel perfonning the proca:iure have been instructed as to the.imp:)rtance of these procedures.
