UNSCHEDULED RELEASE-PG WATER

1.

DESCRIPrICN OF EVENT:

During routine surveillance of a liquid waste release from "B" CDI', a higher than usual release rate was noticed.

Investigation sha.t1ed that a Primary Grade Water (PG) valve was open.

PG water was inadvertently being discharged along with contents of "B" CDT.

This is contrary to T.S. 3.11.A.4 and is rep:,rtable as :per T.S. 6.6.2.b. (2).

PROBABLE CONSEQJENCES AND STATUS OF REDUNDANT SYSTEMS:

Samples of the Pr.irrary Grade water were taken and counted.

The evaluation of the quantity of PG water that was released (74 gallons) and activity detected in the PG water showed the unsampled release to be *within Technical Specifications (71 µCi or.0153% of IIBX. allowed).

In addition, the flow path was through the radiation rronitor and the autanatic function vK)uld have terminated the release if a higher radioactivity set p:,int level had been r-eached.

Therefore, the health and safety of the public were not affected.

CAUSE

Primary Grade water was being used to dilute the contents of Lwrr SA and SB.

The piping arrangement is such that during the release of "B" CDT, sane (74 gallons) of this PG water was discharged via HCV-LW-104A, along with the con-tents of "B" CDT.

Operators were utilizing two separate procedures that were not specifically written for performance at the same time.

IMMEDIATE CORRECTIVE ACTION

The PG valve was secured as soon as a greater than nonral release rate was noticed by the operator. Samples were taken of the PG water for nuclide identification.

None required.

ACTION TAKEN TO PREilENT REaJRRENCE:

Applicable procedures will be revised to include precautions to be aware of the hazard.
