!With the unit at_ 76% of rated power artcf'stable, the recirculation valve, l-RS-6 for the o I 2 i Outside Recirculation Spray pump 'B', was found to be open.

Four 50% capacity Recircula-tion Spray Subsystems are provided, therefore, the unavailability of one of these sub-o I-= I systems will not prevent the Spray system from performing its function.

This is a 0 ! 5 O 16 degraded mode of operation contrary to Tech. Spec. 3.4.A.2 and is reportable per Tech.

Therefore, the health and safety of the public were not affected.

RS VALVE l-RS-6 IN IMPROPER POSITION

DESCRIPTION OF EVENT

With the unit at 76% of rated power and stable, the recirculation valve l-RS-6 for the Outside Recirculation Spray pump 'B' was found to be open.

This event was diSc@vered while performing Checklist 53, specifically designed to detect such abnormal conditions.

This is a degraded mode of operation contrary to Technical Specification 3~4.A.2 and is reportable per Technical Specification 6:6.2.b. (2).

PROBABLE CONSEQUENCES AND STATUS OF REDUNDANT SYSTEMS:

Four 50% capacity recirculation spray subsystems are provided, therefore the unavailability of one of these subsystems will not prevent the spray system from performing its function.

Therefore, the health and safety of the public were not affected.

It should be noted that l-RS-6 is a recirc testing valve only, and that a significant portion of_the design flow of the pump would have been available to the spray ring.

CAUSE

The valve was left open at some time after the Periodic Test 17.3.

The Periodic Test (PT 17.3, Containment Outside Recirc Spray Pumps) was completed on September 3, 1980 and the step placing the valve in the closed position was signed off and verified by a second individual.

4.

IMMEDIATE.CORRECTIVE ACTION:

The recirculation valve, l-RS-6, was closed.

5.

SUBSEQUENT CORRECTIVE ACTION:

None required.

6.

ACTION TAKEN TO PREVENT RECURRENCE:

Quality Control will conduct random verification of valve position following completion of _Periodic Tests.
