This valve is required to close on a loss of offsite power coincident with a loss of coolant accident.

An operator was assigned to manually close the valve if the need arose.

The health and safety of the public were not affected *.

MOV-SH-102B MALFUNCTION

1.

DESCRIPTION OF EVENT

  • while performing Periodic Test 25.1 the motor operated valve, MOV-SW-102B, SW to CC Heat Exchangers, did not cycle.

The motor overload was found to have operated.

This valve protects us from violating T.S. 3.14 and is reportable under T.S. 6.6_.2.b. (2).

2.

PROBABLE CONSEQUENCES AND STATUS OF REDUNDANT SYSTEMS:

This valve is required to close to insure sufficient quantity of water in the intake canal for RS operation.

However, ari operator was assigned to close this valve if it became necessary.

The health and safety of the public were not affected.

3.

CAUSE

The motor windings were degraded as a result of previous exposure to water.

4.

IMMEDIATE CORRECTIVE ACTION

An operator was assigned to close the valve if it became necessary.

5.

SUBSEQUENT CORRECTIVE ACTION:

The motor was replaced and the valve cycled normally.

6.

ACTION TAKEN TO PREVENT RECURRENCE:

None deemed necessary.

7.
