1.

DESCRIPTION OF EVENT

e With the unit at steady state power the discharge tunnel radiation monitor was found out of service during release of 'A' Liquid Waste Test Tank (LWTT).

This is contrary to T.S. 3.11.A.5 and reportable per T.S. 6~6.2.b.(2).

2.

PROBABLE CONSEQUENCES:

'A' UifTT had been sampled prior to being released and all isotopes were within specifications.

Also, the liquid waste monitor was operational during the release; therefore, the health and safety of the general public were not affected *.

3.

CAUSE

The monitor failed because of a defective circuit card in the monitor circuitry.

4.

I}1MEDIATE CORRECTIVE ACTION:

5.

The immediate corrective action was to stop the release.

SUBSEQUENT CORRECTIVE ACTION:
