Feedwater Bypass Valve Malfunction

1.

DESCRIPTION OF EVENT

e During pre-startup testing, the instrument air supply to the Feedwater Bypass Valve (HCV-FW-155B) was secured.

The valve did not fail closed on loss of air pressure as per design.

This is reportable pursuant to Technical Specification 6.6.2.b.2.

2.

PROBABLE CONSEQUENCES AND STATUS OF REDUNDANT SYSTEMS:

Upon safety injection initiation the bypass valves are designed to trip closed, failure to do so could cause excessive cooldown rates until the isolation valves could be closed.

Since the problem was identified prior to startup, the health and safety of the general public were not affected.

3.. CAUSE:

The valve did not close due to a dirty and sticking pilot valve.

4.

IMMEDIATE CORRECTIVE ACTION

The isolation valves for the Feedwater Bypass Valve were immediately closed.

The pilot valve was cleaned and the bypass valve tested operable and returned to service.

5.

SUBSEQUENT CORRECTIVE ACTIONS:

The other two Feedwa ter Bypass Valves were tested and found to operate as re quired.

6.

ACTION TAKEN TO PREVENT RECURRENCE:

None required.

7.

GENERIC IMPLICATIONS:

The other bypass valves were tested with no problems encountered.

Therefore, there are no generic implications.
