i APPENDIX A NOTICE OF VIOLATION JUL O :; 1979 Virginia Electric and Power Company Surry-Power Station, Units 1 and 2 License Nos. DPR-32 DPR-37 Based on the NRC inspection June 11-13, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below.

These items have been categorized as described in correspondence to you

  • dated December 31, 1974.

A.

As required by Technical Specification 6.4.B.1.C, any individual or group of indivduals permitted to enter a high radiation area shall be provided with a radiation monitoring.device which continuously indicates the radiation dose rate in the area.

10 CFR 20.202(b)(3) defines a high radiation area as any area, accessible to personnel, in which there exists radiation... at such levels that a major portion of the body could receive in any one hour a dose in excess of 100 mrem.

Contrary to the above, on June 13 three individuals were observed working in Unit 1 containment in an area with general area radiation levels of 200 mrem/hr without a survey meter.

This is an Infraction.

B.

As required by 10 CFR 20.103(c), equipment used to limit the inhalation of airborne radioactive material shall be used as stipulated in Regulatory Guide 8.15, "Acceptable Programs for Respiratory Protection." Regulatory Guide 8. 15, Section 4.e requires implementation of written procedures for the issuance and proper use of respirators. The facility Respiratory Protection Manual Section 7.2.5.C states Radiation Work Permit (RWP) Numbers will be logged when a respirator is issued.

Contrary to the above, during the week of June 4-11, 1979 thirty-five of seventy-nine respirators issued had errors in logged RWP numbers.

These errors involved logging of expired RWP's and RWP's that did not apply to the indicated work area.

This is a Deficiency.
