DPR-32 DPR-37 We have reviewed your letter of July 3, 1979, in reference to the inspection conducted at Surry Power Station Unit Nos. 1 and 2 on June 11-13, 1979, and reported in IE Inspection Report Nos. 50-280/79-32 and 50-281/79-50. Our responses to the specific infractions are attached.

NRC COMMENT As required by Technical Specification 6.4.B.1.E. any individual or group of individuals permitted to enter a high radiation area shall be provided with a radiation monitoring device, which continuously indicates the r~diation dose rate in the area.

10 CFR 20.202(b)(3) defines a high radiation area as any area, accessible to personnel, in which there exists radiation **** at such levels that a major portion of the body could receive in any one hour a dose in excess of 100 mrem.

Contrary to the above, on June 13, three individuals were observed working in Unit 1 containment in an area with general area radiation levels of 200 mrem/hr without a survey meter~

This is an infraction.

  • The above infraction is correct as stated~

(1) Corrective steps which have been taken and the results achieved:

The _Assistant Supervisor, Health Physics accompanying the inspector instructed the individuals to leave the area and obtain a dose rate m.etet from *the containment personnel hatch area prior to re-entry into that area.

(2) Corrective steps which have been taken to avoid further non-compliance:

A Health Physics Service Desk has been established in an area adjacent to t*he Heal th Physics office. Dose rate meters will be issued at this point to station personnel who will enter high radiation areas.

(3) The date when full compliance will be achieved:

Full compliance has been achieved.

  • NRC COMMENT As required by 10 CFR 20.103(c), equipment used to limit the inhalation of airborne radioactive material shall be used as stipul..... ted in Regulatory Guide 8.15, "Accertable Programs for Respiratory Protection".

Regulatory Guide 8.15, Section 4.e requires implementation of written procedures for the issuance and proper use of respirators.

The facility Respiratory Prot*ection Manual Section 7.2.5.c states Radiation Work Permit (RWP)

Numbers. will be logged when a r~spirator is issued.

  • Attachment, page 2 of 2 Contrary to the above, during the week of June 4-11, 1979, thirty-five of seventy-nine respirators issued had errors in logged RWP numbers.

These errors involved logging of expired RWP's and RWP's that did not apply to the indicated work area.

This is a deficiency.

The above deficiency is correct as stated.

(1) Corrective steps which have been taken and the results achieved:

A Health Physics Service Desk has been established in an area adjacent to the Health Physics office~

Copies of all active radiation work permits (RWP's) are at this desk. The appropriate RWP number will be logged on the appropriate form after review by Health Physics personnel manning this desk.

(2) Corrective *steps which have been taken to avoid further non-compliance:

No further steps are deemed necessary.

(3) The date when full compliance will be achieved:

Full compliance has been achieved
