I Bearing failure with resultant gear malfunction prevented the valve from closing as designed. Parts were replaced and valve was returned to service.

Failure of Valve (MOV-lOOD) to Close on Signal

1.

Description of Event

With the reactor at cold shutdown condition and while attempting.to secure purging, it was noted MOV-lOOD did not close on signal.

Further efforts to shut the valve manually were equally unsuccessful.

2.

Probable Consequences and Status of Redundant Systems:

Since there are two valves (one inside and one outside containment) giving a redundancy to the system, and the second valve functioned properly~

the integrity of the system was maintained.

The health and safety of the public were not affected.

3.

Cause

This malfunction was caused by an internal bearing failure which prevented the valve operator from functioning properly.

4.

Immediate Corrective Action

When manual movement of.the valve proved unsuccessful, a maintenance report was initiated to correct the problem.

5.

Subsequent Corrective Action:

The bearing was replaced and the unit was tested satisfactory after maintenance.

6.

Action Taken to Prevent Recurrence:

This event is considered a random mechanical failure, and no further action is necessary.
