APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Salem Nuclear Generating Station Unit 2 Docket No: 50-311 License No: DPR-75 During an NRC inspection conducted on October 25, 1998 through December 5, 1998, a violation of NRC requirements was identified. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the violation is listed below:
10 CFR 50, Appendix 8, Criterion XVI, °Corrective Action," requires, in part, that corrective actions be taken to preclude the repetition of significant conditions.

Contrary to the above, on September 16, 1998, the NRC identified measuring and test equipment (M& TE) connected to all four operable channels of the reactor protection system (RPS) without appropriate channel separation and electrical isolation, and without an adequate evaluation of the potential impact on the RPS channels. The NRC has identified similar instances of M&TE connected to operable safety-related systems without adequate evaluations. Specifically, PSE&G personnel connected M& TE to an operable emergency diesel generator and to an operable vital battery without evaluating the potential impacts. Corrective actions from these events were ineffective in precluding the most recently identified example.
This is a Severity Level IV violation (Supplement I).
The NRC has concluded that the information regarding the reasons for the violation, the corrective actions taken and planned to correct the violation and prevent recurrence, and the date when full compliance was achieved is already adequately addressed on the docket in LER 50-272/98-015-00dated October 22, 1998, and the enclosed inspection report.
However, you are required to submit a written statement or explanation pursuant to 10 CFR 2.201 if the description therein does not accurately reflect your corrective actions or your position. In that case, or if you choose to respond, clearly mark your response as a "Reply to a Notice of Violation," and send it to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region I, and a copy to the NRC Resident Inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).
If you contest this enforcement action, you should also provide a copy of your response to the Director, Office of Enforcement, United States Nuclear Regulatory Commission, Washington, D.C. 20555-0001.
