NRC INTEGRATED INSPECTION REPORT 50-272/98-11, 50~311 /98-11

NOTICE OF VIOLATION

Dear Mr. Keiser:

On December 5, 1998, the NRC completed an inspection at your Salem 1 & 2 reactor

facilities .. The enclosed report presents the results of that inspection.

During the six-week period covered by this inspection report, your conduct of activities at

the Salem facilities was generally characterized by safety-conscious operations, sound

engineering and maintenance practices, and careful radiological work controls. The

inspectors conducted a focused review of your radiation protection program and found that

you have established an effective program. Activities associated with the shutdown of

Unit 2 to make repairs to a reactor coolant pump seal were performed in a deliberate

manner with clear communications. Additionally, our inspectors noted significant

improvement since our previous assessment of the coordination of Unit 1 steam leak repair

activities.

We are concerned, however, about a violation of NRC requirements that resulted in the

failure to maintain reactor protection system (RPS) channel separation and electrical

isolation. Specifically, as a result of concerns raised by NRC inspectors, your staff

determined that RPS design bases were compromised during data collection activities

occurring simultaneously on all four RPS channels. We have reviewed your licensee event

report (LER) for this event and concluded that corrective actions were reasonable and

comprehensive. However, because corrective actions for previous similar issues were not

effective in preventing recurrence, this violation was cited.

The NRC has concluded that information regarding the reason for the violation and the

corrective actions taken and planned to correct the violation and prevent recurrence is

already adequately addressed on the docket in the aforementioned LER (50-272198-015-

00) dated October 22, 1998, and the enclosed inspection report. Therefore, you are not

required to respond to this letter unless the description therein does not accurately reflect

your corrective actions or your position. In that case, or if you choose to provide

additional information, you should follow the instructions specified in the enclosed Notice.
