APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Salem Nuclear Generating Station Docket Nos: 50-272 50-311 License Nos: DPR-70 DPR-75 Units 1 and 2 During an NRC inspection conducted on March 16, 1997 to April 26, 1997, a violation(s) of NRC requirements was (were) identified. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, tha violation(s) is (are) listed below:
A.
Technical Specification 3.8.2.2 for Salem Unit 2 requires that, in modes 5 and 6 with less than two AC electrical busses and inverters operable and energized, operators establish containment integrity within 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />.
B.
Contrary to the above, on April 8, 1997, with Salem Unit 2 in mode 5 and less than two AC electrical busses and inverters operable and energized, operators did not establish containment integrity within 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />.
Technical Specification 6.11 states that procedures for personnel radiation protection... shall be approved, maintained and adhered to for all operations involving personnel radiation exposure. Procedure NC.NA-AP.ZZ-0024(0), Rev. 7, "Radiation Protection Program," Section 3.6, states that the Principal Health Physicist-Radiological Safety is responsible for ensuring periodic reviews of the ALARA program and periodic assessments of the station radiation protection prowam are conducted.
Contrary to the above, the Principal Health Physicist-Radiological Safety, has not scheduled or provided periodic reviews of. the ALARA program or periodic assessments of the station radiation protection program for at least the last 4 years.
This is a Severity Level IV violation (Supplement 1 ).
Pursuant to the provisions of 10 CFR 2.201, Pubic Service Electric and Gas Company is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region I, and a copy to the NRC Resident Inspector at the f~cility _that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a "Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Your response may reference or include previous docketed correspondence, if the correspondence adequately addresses the required response. 2 received within the time specified in this Notice, an order or a Demand for Information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
