I APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Salem Nuclear Generating Station Docket Nos: 50-272 50-311 License Nos: DPR-70 DPR-75 Units 1 and 2 During an NRC inspection conducted on September 15, 1996 to November 2, 1996, violations of NRC requirements were identified. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the violations are listed below:
A.
10 CFR 50, Appendix 8, Criterion XVI, "Corrective Action," requires, in part, that licensees shall promptly identify and correct conditions adverse to quality, and for significant conditions adverse to quality, the licensee shall also determine the cause, take action to preclude repetition, document the corrective action, and notify appropriate levels of management.
Contrary to the above, the Salem staff did not promptly identify, correct, determine the cause, notify appropriate levels of management, or initiate corrective action to preclude recurrence for the following conditions:
- 1.
From September 15, to October 24, 1996, the operations staff failed to promptly identify and correct significant conditions adverse to quality consisting of tagging process deficiencies, potential equipment damage, and operator procedure non-compliance related to an emergency control air compressor degraded condition.
- 2.
Prior to October 26, 1996, the operating shift failed to document, notify appropriate levels of management, or initiate corrective actions for recurrence of a significant condition adverse to quality consisting of degraded station air compressor piping, and an alarm response procedure non-compliance identified by inspectors on October 23, 1996.
- 3.
From September 3 to October 24, 1996, the operations staff failed to promptly identify, correct, or notify appropriate levels of management of significant conditions adverse to quality consisting of inappropriate Unit 2 operator actions in response to a service water bay depressurization.
This is a Severity Level IV violation (Supplement 1).
B.
Technical Specification 6.8.1 requires, in part, that licensees establish, implement and maintain written procedures covering the applicable procedures in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978. Regulatory Guide 1.33 9612090262 961203 PDR ADOCK 05000272 G

requires written procedures to control operation of safety related plant equipment.
Salem Procedure NC.NA-AP.ZZ-0001, Revision 8, Nuclear Procedure System, step 5.3.7.D requires that implementing procedure steps identified with numbers or letters should be completed in order unless the procedure allows otherwise Contrary to the above, on October 21, 1996, Unit 1 operators did not complete procedure S1.0P-50.KV-0003, Revision 10, 1C4KVVita/Bus Operation, steps 5.2.4 and 5.2.3 in order in that they completed step 5.2.4 prior to performing step 5.2.3, and the procedure did not allow otherwise.
This is a Severity Level IV violation (Supplement 1).
Pursuant to the provisions of 10 CFR 2.201, Pubic Service Electric and Gas Company is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region I, and a copy to the NRC Resident Inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a "Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Your response may reference or include previous docketed correspondence, if the correspondence adequately addresses the required response. If an adequate reply is not received within the time specified in this Notice, an order or a Demand for Information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
Because your response will be placed in the NRC Public Document Room (PDR), to the extent possible, it should not include any personal privacy, proprietary, or safeguards information so that it can be placed in the PDR without redaction. However, if you find it necessary to include such information, you should clearly indicate the specific information that you desire not to be placed in the PDR, and provide the legal basis to support your request for withholding the information from the public.
