NRC INTEGRATED INSPECTION REPORT 50-272/96-15, 50-311/96-15
Dear Mr. Eliason:
On November 2, 1996, the NRC completed an inspection at your Salem 1 & 2 reactor
facilities. The enclosed report presents the results of that inspection as well as several.
violations of NRC requirements.
During the 7-week period covered by this inspection period, your conduct of activities at
the Salem facilities was generally characterized by safety-conscious operations, sound
engineering and maintenance practices, and careful radiological work controls. We are
concerned, however, about several operator failures to adhere to procedures, and the
subsequent ineffective corrective action. We understand that human errors will
occasionally occur. However, the absence of a rigorous evaluation and understanding of
the events during this period gives cause for concern. Notwithstanding, we continue to
observe overall improvement in the performance of both your organization and plant
equipment.
Your staff has shown a high level of performance on the steam generator replacement
project to date. We encourage a continued process of planning, anticipation of problems,
self assessment of results and appropriate corrective actions.
In accordance with 10 CFR 2. 790 of the NRC's "Rules of Practice," a copy of this letter
and its enclosures will be placed in the NRC Public Document Room (PDR). The violations
are cited in the enclosed Notice of Violation and the circumstances surrounding them are
described in detail in the subject inspection report. Please note that you are required to
respond to this letter and should follow the instructions specified in the enclosed Notice
when preparing your response. The NRC will use your response, in part, to determine
whether further enforcement action is necessary to ensure compliance with regulatory
requirements.
