COMBINED INSPECTION REPORT NOS. 50-272/9~-07, 50-311/94-07,

AND 50-354/94-05

This refers to the announced safety inspection conducted by Mr. J. Calvert,

Mr. R. Skokowski, and Mr. A. Della Greca of this office from March 28 through

May 20, 1994, at the Salem and Hope Creek Nuclear Generating Stations in Hancocks

Bridge, New Jersey. At the conclusion of the inspection, the findings were discussed with

those members of your staff identified in the enclosed report.

This inspection was directed toward assessment of areas important to public health and

safety. Areas examined during this inspection were the activities related to the engineering

and technical support provided for the operation of the Salem and Hope Creek stations.

Within these areas, the inspection consisted of selected examinations of procedures and

representative records, interviews with personnel, and observations by the inspectors.

The inspection indicated that the engineering and technical support organizations were

adequately staffed. Improvements were noted in certain areas, including backlog reductions,

design changes and modifications, corrective actions, and engineering management attention

to digital issues. However, two system interface design inadequacies in the Hope Creek

digital feedwater system upgrade contributed to a reactor scram, which indicated that

additional management attention is required to ensure effective design control.

Based on the results of this inspection, certain of your activities appeared to be in violation

of NRC requirements, as specified in the enclosed Notice of Violation (Notice). Specifically,.

no written safety evaluation, according to 10 CFR 50.59, was performed for a change to the

Salem 1, 1A-460V vital bus transformer. The violation is of concern because it was not

identified by your review process, and it indicates a potential for the inadequate application

of your safety evaluation process for small design change packages.

You are required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response. In your response, you should document the

specific actions taken and any additional actions you plan to prevent recurrence. After

reviewing your response to this Notice, including your proposed corrective actions and the

results of future inspections, the NRC will determine whether further NRC enforcement

action is necessary to ensure compliance with NRC regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter, its

enclosure(s), and your response will be placed in the NRC Public Document Room.

The responses directed by this letter and the enclosed Notice are not subject to the clearance

procedures of the Office of Management and Budget, as required by the Paperwork

Reduction Act of 1980, Pub. L. No. 96.511.
