Gentlemen:

Subject: NRC Combined Inspection No. 50-272/90-12; 50-311/90-12

This letter transmits the findings of a special inspection conducted by Messrs.

T. P. Johnson, G. M. Tracy, S. T. Barr, and S. M. Pindale during the period

April 11 - 18, 1990, at the Salem Nuclear Generating Station.

The findings of

the special inspection were discussed with Mr. L. K. Miller of your staff at

the conclusion of the inspection.

The.inspection was conducted to review the circumstances surrounding the mis-

calculation of the safety injection pumps' flow rates in the emergency core

cooling system (ECCS) flow balance verification surveillance procedure.

The

miscalculation resulted in each unit being operated with the flow rates for one

of the two intermediate-head safety injectior pumps exceeding the maximum value

allowed by Technical Specifications.

Although the miscalculation was identified by your staff and reported to the

NRC, we note that a similar miscalculation, involving surveillances for the

high-head charging pumps, was identified by your staff in January 4, 1990.

The

January 4, 1990, event and your corrective actions were also reported to the

NRC (Reference LER No.90-002).

Your corrective actions for the previous event

indicated all similar surveillances were reviewed and no additional examples

were identified.

The identification of a second example of apparent failure to meet Technical

Specification ECCS surveillance requirements raises concerns about the adequacy

of your corrective actions for the previous event.

Consequently, we plan to

conduct an Enforcement Conference with you at the NRC Region I office in the

near future to discuss this matter .
