APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Salem Unit 2 Docket No.
50-311 License No. DPR-75 As a result of the inspection conducted on May 19, 1987 to June 15, 1987, and in accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions, 11 10 CFR 2, Appendix C (Enforcement Policy 1986),
the following violation was identified:
A.
Technical Specification (TS) Surveillance 4.5.2b states each ECCS subsystem shall be demonstrated operable at least once per 31 days by:
- 1.
Verifying that each valve (manual, power operated or automatic) in the flow path that is not locked, sealed or otherwise secured in position, is in its correct position.
- 2.
Verifying that the ECCS piping is full of water by venting the ECCS pump casings and accessible discharge piping high points.
Further, TS 6.8.lc requires that written procedures shall be established, implemented and maintained covering... surveillance and test activities of safety related equipment.
Surveillance Procedure SP(0)4.5.2b provides instructions and checkoff sheets for the performance of TS Surveillance 4.5.2b.
Contrary to the above, on May 20, 1987 it was determined that on April 18, 1987 Surveillance Procedure SP(0)4.5.2b was not properly maintained or implemented resulting in TS Surveillance 4.5.2b not being completed within the last 31 days as required.
This is a Severity Level IV violation (Supplement I) applicable to DPR-75.
Pursuant to the pro~isions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit to this office within 30 days of the date of ~he letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the corrective steps which have been taken and the results achieved; (2) the corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved~ Where good cause is shown, consideration _will be. given to extending. the response time.
