APPENDIX A NOTICE OF VIOLATION Public Service Electric And Gas Company Salem Nuclear Generating Station, Units 1 and 2 Docket Nos.
50-272 50-311 License Nos. DPR-70 DPR-75 As a result of the inspection conducted on February 24-28, 1986, and in ac-cordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), published in the Federal Register on March 8, 1984 (47 FR 9987), the following violation was identified:

10 CFR 71.12 (b)(2) requires, in part, that each licensee comply with the terms and conditions of the Certificate of Compliance (CDC) issued by the NRC in order to deliver an approved shipping package to a carrier for transport.
CDC Number 6601, governing the general licensed use of Model CNSI 8-120 Radioactive Material Shipping Packages, requires, in part, that the packaging be leak tested once every 12 months in accordance with the supplier's application for the CDC (condition 9 of CDC Number 6601).
Contrary to the above, 5 radioactive waste shipments were made in Model CNSI 8-120 shipping packages in April, May, August and October 1985 and the licensee failed to verify in each instance that the packaging had been leak tested during the preceding 12 month period.
This is a Severity Level V violation.

Pursuant to the provisions of 10 CFR 2.201, Public Service Electric And Gas Company is hereby required to submit to this office, a written statement or*
explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending this re-sponse time.
