APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Salem Unit 1 Docket No.
50-272 License No. DPR-70 As a result of the inspection conducted on September 22, 1985, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C) published in the Federal Register on March 8, 1984 (47 FR 8583), the following violation was identified:

Technical Specification 6.8 states that...

- Regulatory Guide 1.33, February 1972, Appendix A, Item F.1 recommends procedures for combating emergencies and other significant events including...
11 Loss of Coolant (including significant PWR steam generator leaks) (inside and outside primary containment) (large and small, including leak-rate determination).
11 Contrary to the above, during the inspection and review of Salem Unit -1 Reactor Coolant System (RCS) leak Rate determinations made on September 22, 1985, from 5:46 p.m. to 8:53 p.m., Procedure SP(O) 4.4.6.2(d), RCS Water Inventory Balance, was inadequately established in that primary leakage occurring inside the containment was classified improperly. This hindered entry into the Limiting Condition for Operation for Technical Specification 3.4.6.2 for events where the 11 unclassified 11 leakage would have increased the unidentified leakage above 1.0 gallon per minute.
This is a Severity Level IV violation (Supplement I) applicable to DPR-70.
Pursuant to the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the corrective steps which have been taken and the results achieved; (2) the corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending the response time.
