Salem Generating Station, Unit 2 Public Service Electric & Gas Company Hancock's Bridge, New Jersey 08038 IDENTIFICATION OF OCCURRENCE:

Greater than one Safety Injection Pump operable with the Reactor Coolant System cold leg temperature < 312°F CONDITIONS PRIOR TO OCCURRENCE:

Mode 5 Cold Shutdown DESCRIPTION OF OCCURRENCE:

While in cold shutdown, it was discovered that both Safety Injection Pumps had been made operable at the same time for surveillance test-ing.

Technical Specification 3.5.3 requires that a maximum of one Safety Injection Pump shall be operable whenever the temperature of one or more of the RCS cold legs is less than or equal*to 312°F.

The pumps were operable at the same time for a period of approxi-mately 25 minutes.

The flow paths, however, were inoperable due to closed discharge valves into the RCS System.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Personnel Error ANALYSIS OF OCCURRENCE:

When the reactor coolant temperature is less than or equal to 312°F, the Safety Injection Pump breakers are racked out and tagged to remove a ma 1jor credible source of an RCS pressure transient.

Technical Specification 3.5.3 allows a maximum of one Safety Injection Pump to be operable in this condition.

Since the surveillance was due on both pumps, the tags were released and the supervisor instructed an operator to rack in one pump circuit breaker at a time.

The operator misunderstood the importance of the instructions and performed his job in what he deemed to be a more efficient manner by racking in both circuit breakers at the same time.

The supervisor, by releasing the tags on both pumps simultaneously, gave up the administrtive control that he as the responsible licensed.

CORRECTIVE ACTION

The supervisor was advised of the error and instructed not to release both pumps for service when only one shall be operable.
