Inoperable Power Operated Relief Valves Limit Switches CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 Low Power Physics Testing DESCRIPTION OF OCCURRENCE:

During a review of equipment qualification records, it was determined the limit switches for the power operated relief valves were not seismically and environmentally qualified as stated in our letter of

  • March 28, 1980.

The problems associated with the limit switches were

~eported to the Assistant Director for Licensing by phone on August 22, 1980.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Design Erro*r ANALYSIS OF OCCURRENCE:

Because of an internal communication problem, our response to NUREG 0578 in Section 2.l.3A was incorrectly stated.

The limit switches are not seismically or environmentally qualified.

CORRECTIVE ACTION

Design Change Request 2EC0818 has been approved for implementation, and the limit switches will be replaced at the next cooldown with Namco Snaplock switches, model EA180.
