Steam Generator Flow and Pressure Instruments Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Mode 3, Prior to Initial Criticality DESCRIPTION OF OCCURRENCE:

During a Reactor Coolant System heatup, 21 Steam Generator Steam Flow Channel II, 22 Steam Generator Steam Flow Channel I, 23 Steam Generator Steam Flow Channel I and 23 Steam Generator Pressure Channel II were declared inoperable at 1220 when they failed to respond properly.

Action Statement 3.3.2.1 was entered which requires with Engineered Safety Features Actuation System instru-mentation channel inoperable, operation may proceed until the next required Channel Functional Test, provided the inoperable channel is placed in the tripped condition within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Procedural inadequacy, in that the Main Steam System did not need to be lined up by procedure until the Turbine Generator was to be rolled.

ANALYSIS OF OCCURRENCE:

In order to ensure all protection channels are available, the Main Steam System should be in the proper valve lineup prior to com-mencipg *the plant heatup.

CORRECTIVE ACTION

The isolation valve was opened to place the instruments into an operable condition and the action statement terminated at 1926.

LER 80-09-03L Qn July 10, 1980, a complete valve lineup of the Main Stearn System was completed.

The overall operating procedure has been changed to require the Main Steam System to be in operation prior to commencing the plant heatup.
