Pressurizer Pressure Channel Failures CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 Prior to Initial Criticality DESCRIPTION OF OCCURRENCE:

During a Reactor Coolant System heatup when pressurizer pressure instrumentation should have indicated on scale at 1700 psig, it was determined that Channels I and II failed to respond.

At 0700 hours0.0081 days <br />0.194 hours <br />0.00116 weeks <br />2.6635e-4 months <br />, the channels were declared inoperable and the associated bistables were placed in a tripped condition.

With two channels inoperable, Action Statement _3.0.3 was entered which requires that the unit shall be placed in at least Hot Standby within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />, in at *least Hot Shutdown within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in at least Cold Shutdown within the following 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> unless corrective measures are completed that permit operation under the permissible Action Statement for the.specified time interval as measured from initial discovery or until the reactor is placed in a mode in which the specification is not applicable.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure and Personnel Error ANALYSIS OF OCCURRENCE:

Channel I had a broken fuse cap which prevented the completion of the circuit for proper operation.

Channel II's valve lineup for the instrument sensor was improper

  • _and apparently has been since prior to licensing of the unit.

The valve found shut was the panel isolation valve which is not used during operation of the plant or for any maintenance of the channel transmitter.

CORRECTIVE ACTION

The fuse ~ap was replaced on Channel I completing the circuit.

The panel isolation valve was opened and the channel responded prop-erly.

Since the panel isolation valve is not used during operation, no further corrective action is necessary.

It is an isolated instance.

The channels were tested satisfactorily and the Action Statement terminated at 0903 hours0.0105 days <br />0.251 hours <br />0.00149 weeks <br />3.435915e-4 months <br />.
