REPORT
NOS. 50-259/89-18,
50-260/89-18,
AND
50-296/89-18)
This refers
to the Nuclear
Regulatory
Commission
(NRC) special
reactive
team
inspection
conducted
by
D.
R. Carpenter
and others
on
February
10 -
May 22,
1989
and June
12-14,
1989.
The purpose of this inspection
was to follow up on
the issues
raised in
NRC Inspection
Report 89-04 issued
on January
30,
1989,
and
your response
to that report dated
March 1,
1989.
These
issues
were related to
your initiating the Unit 2, Cycle
6 core reloading without adequate
monitoring
of the core
neutron flux.
At the conclusion of the inspection
the findings
were discussed
on July 10,
1989, with those
members of your staff identified in
the enclosed
inspection report.
Areas
examined
during
the
inspection
are
identified in the report.
Within
these
areas,
the inspection
consisted
of selective
examinations of procedures
and representative
records,
and interviews with personnel.
The inspection
findings indicate that certain activities
appeared
to violate
NRC requirements.
The violations
and references
to pertinent requirements
are
described
in the
enclosed
inspection
report.
The elements
to
be included in
your response
to the enclosed
Notice of Violation are described
in the Notice.
As stated
in our January
30,
1989,
letter to you transmitting
Inspection
Report 89-04,
we have evaluated
these
issues
to determine if escalated
enforce-
ment action is warranted.
We have confirmed that all of the required refueling
interlocks were in place
and operable,
and
even
though
the core
neutron flux
was
not adequately
monitored
during the initial refueling activities, that
these conditions did not significantly reduce the margin of safety.
Therefore,
escalated
enforcement
action
is
not
warranted.
This conclusion
does
not
lessen
our
concern
about
resolving
the
problems
that
resulted
in
your
initiating the Cycle
6 reload without adequate
core neutron flux monitoring.
We
have
examined
these
issues
further and
have
concluded
that the procedures
that controlled
and guided the refueling" activities are of concern
to us.
It
appears
that
a
thorough
multidisciplinary technical
review
comparing
the
procedures
with one another,
your. requirements,
the technical
specifications,
and industry standards
and guidelines
may have precluded this event.

have contributed to
a
number of problems.
We
are
especially'oncerned
because
these
pro'cedures
have
been

through the Procedures
Upgrade
Program .which included
walkdowns
and
" Verification
prior, to
implementation.
The
adequacy
of
procedures
is
idhntified 's
an
unresolved
item in par'agraphs
5
and
8 of
the
enclosed
inspection report.
As discussed
in our exit meeting
on July 10,
1989,
we will
meet with you as
soon
as you are ready to discuss
the meas'ures
you are taking
.to
ensure
that
your
station
procedures
are
adequate
to
support
Unit
2
startup.
In-.accordance
with Section
2.790
of the
NRC's "Rules of Practice,"
Part 2,
Title 10,
Code of Federal
Regulations,
a copy of this letter and its enclosures
will be placed in the
NRC Public Document
Room.
The responses
directed
by this letter and its enclosures
are not subject to the
clearance
procedures
of the Office of Management
and Budget as required
by the
Paperwork
Reduction Act of 1980,
