Conditions Prior to Occurrence: Unit 1 97% steady state operation Unit 2 100% steady state operation Unit 3 100% steady state operation Action specified in the Technical Specification Surveillance Requirements met d~~dl 3A, 38, and 3C diesel generators proved operable. Apparent Cause of Occurrence: Restriction in the cooling water. piping upstream and/or downstream from the-heat exchanger. ~Anal sis of Occurrence; No danger to the health or safety of the public,. no resulting significant chain of

events, no release of activity, and no damage to plant or equipment.

Corrective Action

Heat exchanger was removed, cooling water lines flushed, and the unit performed normally. A system, flushing evaluation is being performed. Failure Data: N/A
