Defective hydrogen sensor.

Corrective Action

Replaced hydrogen sensor.

Design change request.

1194 will replace the General Electric hydrogen monitoring system with a system whose sensors are external to the primary containment.

Primary containment modification is being completed on unit 3 during the present refueling outage.
