Operations personnel observed erratic indications on the torus hydrogen sensor H2M-76-38. The system was declared inoperable.

Conditions Prior to Occurrence:

Normal operation.

Action specified in the Technical Specification Surveillance Requirements met duo I. tao~arable a ~ui ment. Describe.'

30-day operating limit was established in accordance with T.S. 3.7.H.4.

A~)arent Cause of Occurrence:

Defective hydrogen sensor.

~Anal sis of Occurrence:

None Corrective Action:

Replaced hydrogen sensor. Design change request. 1194 will replace the General Electric hydrogen monitoring system with a system whose sensors are external to the primary containment. Primary containment modification is being completed on unit 3 during the present refueling outage.
