During the running of the RCIC monthly SI 4,5,F,l.d and e, the operator noticed that there was no speed indication, Conditions Prior to Occurrence:

The unit vas starting up from an outage and was at 43%,

Action specified in the Technical Specification Surveillance Requirements met due to ino erable e ui ment.

The SX was stopped and an urgent TR was written to Electrical Maintenance. 'he SX vas then completed after repairs were made, parent Cause of Occurrence:

The vires which connected the magnetic pickup to the EG~M box had broken, This caused the tachometer to read zero, Anal sis of Occurrence:

There vas no speed indication due to the wires being broken at the magnetic

pickup,
