Reportable Item Shearon Harris Nuclear Power Plant (SHNPP).
Omission 'of Protective Coating and Existence of Damaged Coating on the Unit 1 Containment Liner.
DESCRIPTION On or about September 16, 1980, it was noticed by site inspection personnel that a section of the Unit 1 Containment Liner plate, adjacent to the refueling slab (a two inch gap exists in this area between the liner plate and the slab) had not received the final nuclear coatings required, prior to placing the concrete slab.
In addition in some areas the coating that existed (prime coat only) had been damaged and also had not been repaired.
The section of liner plate in question is approximately five (5) feet in height and sixteen (16) feet long.
SAFETY IMPLICATIONS I
The liner plate in the containment building furnishes no structural value for the building, but rather serves as an impervious barrier for containing any possible type of radioactive contamination/release that may exist inside the building.
For this reason, the liner plate has to be protected against corrosion.
Considering the worst possible environment that could exist inside the containment for an extended period (30 to 40 years) the possibility existed that the liner plate, without the protective coating, could have had a corrosion rate which would cause its wall thickness to fall below the minimum allowable.
THE CAUSE OF THE PROBLE'4/PREVENTATIVE ACTION The cause of the problem was that the concrete placement proceeded past the point where the liner coating could be applied properly as a result of inspectors not being fully aware of the coating requirements of the"liner plate.
The Inspectors saw steel that had a painted surface, although sections were acceptable, and did not really understand what requirements were involved in coating the plate.
Inspection personnel have been advised of the function of the liner plate and the manner in which it must be coated.
CORRECTIVE ACTION FCR-AS-287 was written to allow the coating to be applied without any type of surface preparation and the second prime and top coats to be applied using best possible technique available since there was only a 1 1/2" to 2" gap in which to work.
Inspection criteria would be waived as a result of the location.
Since the liner plate already had a prime coat, which was in fair condition, the Engineer approved this request.
A special spray nozzle (1 1/4" diameter with a 360 application)'as procurred to aid in the coating.
An attempt was made to scarify the surface to improve paint adhesion (this consisted of using steel brushes and a special sandblasting tool that was 1 3/4 inches in diameter).
Not all the surface was scarified but what could be reached was.
After preparing the
- surface, environmental conditions were established around the area and the coating operation proceeded and has now been satisfactorily completed.
