Siemens-Allis
- Company, West Allis, Wisconsin Nature of Deficienc In May, 1980, Siemens-Allis Company (West Allis, Wisconsin) shipped 34 switchgear sill channels of a size recommended by them at that time to the CPGL site on Purchase Order NY-435113, Non Class 1E Seismically-Designed Switchgear.
In August, 1980, while reviewing Siemens-Allis'eismic report for safety-related switchgear (NY-435112 This report covers the seismically-designed items on NY-435113),
Ebasco noted that the required depth of embedment of the sill channels was changed by Siemens-Allis without prior consultation with Ebasco due to seismic considerations.
Ebasco's review of the Siemens-Allis report verified the necessity of the above change and determined that the section modulus of the sill channels was inadequate, resulting in an addi-tional revision to the sill channel design.
At the same time, Ebasco advised Siemens-Allis that the loading configuration at the rear channel required larger-size anchor bolts (i.e., 1-inch versus the previous 5/8-inch bolts) for embedding the rear sill channels.
Siemens-Allis was therefore required to revise the anchor bolt holes in the rear channels accordingly.
- Thus, two deficiencies in the same component were evaluated:

The undersize embedment depth of the sill channels shipped to the site.
The undersize bolt hole dimension of the rear sill channels (the rear channels were never shipped to the site with an under-size bolt hole; the problem was corrected prior to shipment.)
No safety-related sill channels or switchgear had been shipped at that time on NY-435112.
However, neither Ebasco nor Siemens-Allis notified CP&L formally that the sill channels shipped on NY-435113 should be put,on "hold" at the site pending approval of the seismic report.

Since installation of the channels was not
- imminent, Ebasco proceeded to work with Siemens-Allis to obtain the correct equipment.
The failure to identify the incorrect sill channels that were shipped to the site for use in a seismically-designed application could have resulted in the improper installation of the switchgear.
Date Problem Occurred:

December 18, 1980 Ebasco Services (R. Weronick, X. Pique) was notified by CP&L (R. Cothren) of this deficiency.
Date Problem Re orted:
December 22, 1980 CP&L (N. J. Chiangi) notified the NRC Region II (B. Cochran) that this item was potentially reportable under 10CFR50.55(e).

January 21, 1981 CP&L (N. J. Chiangi) notified the NRC Region II (J.
Rausch) that this item was reportable under 10CFR50.55(e).
It should be noted that this item was also evaluated and found to be reportable under 10CFR, Part 21.
The deficiency is limited to 1 5/8" sill channels shipped'n P.O. NY-435113.
These channels have an embedment depth of 1 5/8" instead of the required 2 1/2" embedment depth.
Safet Im lication:
Failure of the sills by pulling out of the concrete channels would allow the switchgear component to either fall over and possibly damage safety-related equipment or allow the switchgear component to be fxee to move about'uring a seismic event and thereby jeopardize the operation of safety-related equipment located in the vicinity.
Failure of the sill channels due to the under-size anchor bolting would have a similar safety implication.
Reasons Deficienc is If left uncorrected, either deficiency by itself could cause the failure of seismically-designed components,'which in turn could cause damage to safety-related equipment.
Corrective Action:
The incorrect (1 5/8") sill channels have been placed on "hold" at the Sheaxon Harris site.
No seismically-designed switchgear has been installed using the incorrect sill channels.
The correct (2 1/2")'ill channels were shipped by Siemens-Allis Company on December 19, 1980.
No rear channels with undersize bolt holes have been shipped at any time.
This problem is considered to be an isolated case'-which should not recur.
Siemens-Allis Company is aware of this problem and has been requested to take the necessary action to prevent similar occurrences.
