In reference to your letter of November 3, 1980, referring to RII: GFM 50-400/80-22, 50-401/80-20, 50-402/80-20, 50-403/80-20, the attached is Carolina Power and Light Company's reply to the deficiencies identified in Appendix A. It is considered that the corrective and preventive actions taken will be satisfactory for resolution of this item, once completed.

INFRACTION Condition Reported:
As required by Criterion'X of Appendix B to 10CFR50, as implemented by Carolina Power and Light Company's (CP5L)
PSAR, Section 1.8.5.9 and CP&L's Corporate QA Program Part I, Sections 6.2.3, and 7,3.4, measures shall be established to assure welding is controlled in accordance with applicable standards.
Contrary to the above, piece number 10 on pipe hanger number CC-H-911 was not welded in accordance with the weld symbol shown on its installation drawing; the installation drawing for pipe hanger CC-H-388 and CC-H-911 did not correctly depict the welds to be applied.
Workers and foreman who were responsible for welding pipe hangers did not understand the meaning of some of the installation welding symbols.
Welding ins'pection personnel were not documenting the pipe hanger olds that were not as depicted on their. installation drawings.
Corrective Steps Taken and Results Achieved:
The nonconforming condition listed in the Notice of Violation was reported in Deficiency and Disposition Report No. 457 for purposes of control and resolution.
The site Quality Assurance unit is in process of reinspecting each seismic and/or safety related hanger installed or par tially installed prior to dis-covery of the problem.
Workmanship errors are being corrected by either reworking the hangers in the field or writing permanent waivers requesting approval from the Architect Engineer (A/E) for hangers to remain as-is.
Completed hangers that are reworked will be reinspected for compliance wi,th drawings.
The hanger drawings are being reviewed for correctness of weld symbols prior to release for rework and/or reinspection.
Incorrect symbols identified will be reported to the A/E and the hanger supplier for correction.
Corrective Steps Taken to Avoid Further Noncompliance:
Weld symbol identification training classes were conducted and attended by field craft personnel and Quality Assurance inspectors.
Craft personnel were instructed in the requirements of welding the hanger only as detailed on the drawings.
There has been a documented 43% reduction in the overall reject rate for installation of new hangers.
The tabulation of hangers welded and inspected since the retraining classes were held was to permit evaluation of the re-training effectiveness.
The data accumulated to date would indicate that the retraining sessions were highly effective.
Supervisory checks of in-spection work indicate that hangers are getting a "tight" inspection and that the inspectors are following the installation drawings and approved FCR's closely.

The hanger supplier and the A/E have agreed to perform more thorough checks of weld symbols prior to release of drawings for construction.
Site Melding Department personnel will also be checking drawings for weld symbols prior to release to the, field.

Full compliance will be considered to have been achieved once the reinspection of hangers is complete, including applicable evaluations and/or required field rework.
Full compliance is anticipated to be complete on May 1, 1981.
