APPENDIX A NOTICE OF VIOLATION Carolina Power and Light Company Shearon Harris Facility License No.
CPPR-158 Based on the NRC inspection October 15-18, 1979, certain of your 'activities were apparently not conducted in full compliance with NRC requirements as indicated below.
These items have been categorized as described in correspondence to you dated December 31, 1974.
As required by Criterion V of Appendix B to 10 CFR 50 and as implemented by PSAR Section 1.8.5.5, activities affecting quality shall be accomplished in accordance with procedures.
Procedures applicable to control of welding electrodes used in construction of the containment liner are included in Chicago Bridge and Iron's "Contract equality Assurance
- Handbook, Contract 84031/2, Div. 4".
Section 8 of this document requires all welding material at the jobsite to be stored in locked storage areas or locked electrode ovens.
Contrary to the above, on October 15,
- 1979, a welding electrode return oven was found unlocked and unattended.
This is a deficiency.
