REPORT
NO. 50-400/89-35)
This refers to the Nuclear Regulatory
Commission
(NRC) inspection
conducted
by
S. Sparks
on
December 4-8,
1989.
The
inspection
included
a
review of
activities authorized for your Shearon
Harris facility.
At the conclusion of
the inspection,
the findings were discussed
with those
members of your staff
identified in the enclosed
Inspection
Report.
Areas
examined
during the inspection
are identified in the report.
Within
these
areas,
the inspection
consisted
of selective
examinations
of procedures
and
representative
records,
interviews with personnel,
and observation
of
activities in progress.
The inspection
findings indicate that certain activities
appeared
to violate
NRC requirements.
The violation, references
to pertinent
requirements,
and
elements
to be included in your response
are described
in the enclosed
Notice
of Violation.
We
are
concerned
with the
implementation
of your inservice
testing
of
valves.
The
omission of check
valves
from your inservice testing
program,
inadequate
check valve exercising,
and failure to test the fail-safe function
of power operated
valves indicate your. review of Generic Letter 89-04 was not
thorough in the areas
of proper
scope of testing,
and
NRC staff positions
on
acceptable
testing methods.
An additional
review of the deficiencies
and
NRC
staff positions
identified in Generic Letter 89-04
should
be
performed
to
assure
the operational
readiness
of appropriate plant valves.
In your response
to the violation, please
describe
actions to be taken to insure the
scope of
Generic Letter 89-04 is fully reflected in your inservice testing program.
In addition,
weaknesses
were identified in your motor operated
valve program
associated
with the proper selection, setting,
and maintaining of valve operator
switches.
These
weaknesses
indicate
the
need for additional
management
attention
to 'insure
safety-related
motor operated
valves will function
as
required.

The
inspection
also
identified that
your
interpretation
regarding operability of power operated relief valves
does
not
agree 'ith the
NRC position.
A re-evaluation
of your. interpretation
is
warranted.
In accordance
with Section
2.790 of the
NRC's
"Rules of Practice,"
Part 2,
Title 10,
Code of Federal
Regulations,
a copy of this letter and its enclosures
will be placed in the
NRC Public Document
Room.
