ENCLOSURE NOTICE OF VIOLATION Carolina Power and Light Company Shearon Harris Unit 1 Docket No. 50-400 License No.
NPF-63 During the Nuclear Regulatory Commission (NRC)
Inspection conducted on June 21 - July 20, 1989, violations of NRC requirements were identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"
10 CFR Part 2, Appendix C (1988), the violations are listed below:
A.
10 CFR 55.53(e) requires a Senior Operator to perform the functions of a Senior Operator for a minimum of five 12-hour shifts per calendar quarter in order to maintain an active license.
10 CFR 55.53(f) states that if part (e) is not met, the facility licensee shall certify the qualifications and status of the licensee as current and that the licensee has completed a minimum of 40 hours4.62963e-4 days <br />0.0111 hours <br />6.613757e-5 weeks <br />1.522e-5 months <br /> of shift functions under the direction of a Senior Operator pr ior to resuming license duties.
Contrary to the above, during the second quarter of 1989 a Senior Operator performed the functions of a Senior Operator for four and one-third 12-hour shifts instead of the required five 12-hour shifts.
The Senior Operator subsequently resumed license duties for three consecutive days in the third quarter of 1989, without obtaining certification as required by 10 CFR 55.53(f).
This is a Severity Level IV violation (Supplement I).
B.
10 CFR 50 Appendix B, Criterion V, Instructions, Procedures, and Drawings, and the Carolina Power and Light Company Corporate guality Assurance
- Program, Section 6, requires that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings and shall be accomplished in accordance with these instructions, procedures or drawings.
Contrary to the above, activities affecting quality were not accomplished in accordance with instructions, procedures or drawings in that the containment spray system NAOH tank outlet flow orifice, FE 7152, was installed backwards while performing maintenance Work Request 86-AG(M1 and the gC hold point for verification of flow direction was initialed as acceptable.
This is a Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Carolina Power and Light Company is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN:
Document Control Desk, Washington, DC 20555, with S9090S K 0'00400 0i9S S90S2i Q
Carolina Power and Light Company Shearon Harris Docket No. 50-400 License No. NPF-63 a copy to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector, Shearon Harris, within 30 days of the date of the letter transmitting this Notice.
This reply should be clearly marked as a "Reply to a Notice of Violation" and should include for each violation:
(1) admission or denial of the alleged violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results
- achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending the response time.
If an adequate reply is not received within the time specified in this notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken.
