SUBJECT: NRC INSPECTION REPORT 50-528/98-05; 50-529/98-05; 50-530/98-05 AND
Dear Mr. Levine:
From May 31 through July 11,*1998, an inspection was conducted at your Palo Verde Nuclear
Generating Station, Units 1, 2, and 3, reactor facilities. The enclosed report presents the scope
and results of that inspection.
During the 6-week period covered by this inspection, your conduct of activities at the Palo
Verde facilitywas generally characterized
by safety-conscious
operations, sound engineering
and maintenance practices, and careful radiological controls; however, one violation of NRC
requirements was identified. The violation resulted from failure of personnel to maintain the
configuration controls for a locked safety-related valve, as required by procedure.
The violation is cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding this violation are described in detail in the enclosed report.
Please note that you
are required to respond to this letter and should followthe instructions specified in the enclosed
Notice when preparing your response.
The NRC will use your response,
in part, to determine
whether further enforcement action is necessary
to ensure compliance with regulatory
requirements.
In accordance
with 10 CFR 2.790 of the NRC's "Rules, of Practice," a copy of this letter, its
enclosure(s), and your response willbe placed in the NRC Public Document Room (PDR).
