SUBJECT:
NRC INSPECTION REPORT 50-528/96-16; 50-529/96-16; 50-530/96-16 AND
Dear Mr. Levine:
An NRC inspection was conducted
October 6 through November 16, 1996, at your Palo
Verde Nuclear Generating Station, Units 1, 2, and 3 reactor facilities. The enclosed report
presents the scope and results of that inspection.
During the 6-week period covered by this inspection, your conduct of activities at the Palo
Verde facility was generally characterized
by safety conscious operations,
sound
engineering
and maintenance
practices,
and careful radiological controls; however,
a
violation of NRC requirements
occurred.
The violation involved two examples of
operations
personnel failing to follow procedure requirements
during the preparation for
draining the reactor coolant system to a midloop condition.
Although these examples were
identified by your. staff, similar problems were identified in NRC Inspection
Reports 50-530/94-38 and 50-528,529,530/95-18,
and are of concern since the
corrective actions did not prevent this recurrence.
The violation is cited in the enclosed
Notice of Violation (Notice) and the circumstances
surrounding this violation are described
in detail in the enclosed report.
Please note that
you are required to respond to this letter and should follow the instructions specified in the
enclosed
Notice when preparing your response.
The NRC will use your response,
in part,
to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.
One unresolved
item was identified concerning the reactor coolant pump lubrication oil
collection system flexible covers.
This matter is discussed
in Section F2.1 of the enclosed
inspection report.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter,
its enclosures,
and your response
will be placed in the NRC Public Document Room (PDR).
To the extent possible, your response
should not include any personal privacy, proprietary,
or safeguards
information so that it can be placed in the PDR without redaction.
