SUBJECT:
NRC INSPECTION REPORT 50-528/96-13; 50-529/96-13; 50-530/96-13 AND
Dear Mr. Levine:
An NRC inspection was conducted August 25 through October 5, 1996, at your Palo
Verde Nuclear Generating Station, Units 1, 2, and 3 reactor facilities. The enclosed report
presents
the scope and results of that inspection.
During the 6-week period covered by this inspection, your conduct of activities at the Palo
Verde facility was generally characterized
by safety conscious
operations,
sound
engineering
and maintenance
practices,
and careful radiological controls; however,
a
violation of NRC requirements
occurred.
The violation involved three examples of plant
personnel
failing to follow a procedure describing the requirements for the control of doors
with design basis functions.
Although these examples were identified by your staff, a
similar problem was identified in NRC Inspection Report 50-528, 529, 530/96-07, and the
actions initiated to prevent recurrence were ineffective.
Additionally, other issues
discussed
in this report identify the need to provide management
attention to the control
and testing of essential ventilation boundaries.
The violation is cited in the enclosed
Notice of Violation (Notice) and the circumstances
surrounding this violation is described
in detail in the enclosed report.
Please note that you
are required to respond to this letter and should follow the instructions specified in the
enclosed
Notice when preparing your response.
The NRC will use your response,
in part,
to determine whether further enforcement action is necessary
to ensure compliance with
regulatory requirements.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of this letter,
its enclosure(s),
and your response
will be placed in the NRC Public Document Room
(PDR). To the extent possible, your response
should not include any personal privacy,
proprietary, or safeguards
